Clinician (LMSW, LPCA, LMFTA) - PHP
Job Summary
Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Adult and Child/Adolescent partial hospitalization program. Clinical services include individual, group and family therapy. Inter-agency collaboration with and exposure to school social workers, principals, DCF, and court systems.
Key Responsibilities
- Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
- Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning.
- Maintains timely patient-centered documentation that reflect the patient and client systems’ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.
Qualifications
- Master’s degree in social work, counseling, or marriage and family therapy or related human service field.
- Previous experience in a healthcare setting required, preferably working with a mental health and/or chemical dependency population.
- Current Connecticut LMSW, LPC-A, LMFT-A.
- Excellent communication skills, with the ability to establish relationships within and outside their entity.
- Is cognizant of health-related social needs and cultural competence.
- Recognizes and addresses health disparities-inequalities and injustices directed toward clients, related to access to care and provision of health services.
Additional Compensation
Additional compensation provided. Behavioral Health Clinicians are eligible for a Sign On Bonus up to $2,500.00, in accordance with program guidelines***
Schedule
Schedule: Monday - Friday; 8:00 AM to 4:30 PM. This position floats to any Natchaug clinical site as needed; including, Vernon, Enfield, Dayville, Danielson, Mansfield, Norwich, and Groton.
