Clinician III- Group Facilitator

About the Role

This position coordinates and delivers clinical behavioral health services to meet the wellness and recovery needs of consumers. Services may include resource development, assessments, counseling (individual, group, or family), and facilitating consumer groups.

Responsibilities

• Conduct initial and ongoing diagnostic assessments, collecting, analyzing, and recording bio-psychosocial information.
• Formulate diagnoses and develop treatment plans based on issues identified by the consumer and clinician.
• Provide individual, family, and group counseling as needed.
• Make collateral contacts and maintain communication with clients, including outreach for hard-to-engage clients.
• Develop formal aftercare plans for clients completing treatment stages and prepare discharge summaries.
• Facilitate consumer groups.
• Maintain clinical documentation in Cerner (electronic health record), including assessments, diagnoses, treatment plans, progress notes, collateral contacts, and termination summaries.
• Provide case management services, such as preparing reports for external entities, community linkages, home-based counseling, benefits assistance, referrals, and client advocacy.
• Spend 65% of time in face-to-face consumer contact and submit productivity reports to supervisors.
• Submit schedules, statistical reports, and clinical reports within prescribed timelines.
• Complete documentation in Cerner within 24 hours of service delivery.
• Participate in individual and peer supervision as required by program needs; consult with program psychiatrists/APNs for treatment plans.
• Engage in the Quality Improvement process and specific program initiatives.
• Interface with medical, vocational, residential, and other program resources as necessary.
• Work as part of a team to provide services outside of primary caseload when needed.
• Supervise interns fulfilling fieldwork requirements as needed.
• Utilize supervision and training to enhance professional competence.
• Perform duties professionally and ethically, adhering to agency, licensing, COA, and professional standards.
• Perform other related duties as assigned.
