What You Will Do
- Celebrate Individuals: Conduct Clinical team report to assess decision-making capacity and functional abilities for individuals with intellectual disability referred for guardianship evaluations.
- Collect Information: Conduct interviews with individuals served, family members, providers, and other relevant collateral contacts to gather information related to functioning and support needs.
- Review Documentation: Review relevant clinical, educational, medical, and service documentation to inform evaluation findings.
- Collaborate: Work collaboratively with a licensed psychologist to complete multidisciplinary guardianship evaluations.
- Provide Recommendations: Provide professional recommendations regarding guardianship, limited guardianship, or alternative decision-making supports when appropriate.
- Prepare Reports: Prepare clear, thorough written evaluation reports that meet Massachusetts court requirements for guardianship proceedings.
- Consult: Participate in case consultation with the psychologist and referring agencies as needed.
