Clinician Field Mentor Nurse Practitioner/ Physician Assistant (Full-time) - Central

Position Summary

The Clinician Field Mentor is an opportunity for experienced clinicians to mentor and coach Signify Health's team while completing In Home Evaluations (IHEs).

What will you do?

• Complete In-home Visits, including IHEs, Focused Visits, and Video Visits.
• Test new Diagnostic & Preventive Services (DPS) and clinical workflow changes.
• Provide 40 hours of availability weekly, with one assigned day/week for coaching and mentorship activities.
• Offer 1:1 support to new and active clinicians, including virtual and in-person shadowing opportunities.
• Facilitate real-time support for in-field clinicians and provide feedback on clinical workflows and DPS.
• Escalate clinical and operational issues to Clinical Leaders and Operations Managers.

Mentorship

• Provide dedicated guidance to new hire clinicians during their first 90 days.
• Facilitate virtual and in-person shadowing opportunities for new clinicians.
• Offer best practice tips to new clinicians on clinical workflows, DPS, etc.
• Offer Group Support to active clinicians on new features/DPS/updates of a clinical nature.

Additional Duties and Responsibilities

• Meet regularly with Clinical Leaders for support and training.
• Engage with regular manager, team, and clinical meetings.
• May attend special events, as needed.
• Available to provide mentoring and coaching support, including rotating evenings and weekends, as assigned.
• Maintain training, affiliation, and compliance with Video Visit requirements.
• Maintain training and complete all diagnostic and preventive service (DPS) products as directed.
• Maintain all required supplies and associated diagnostic equipment related to DPS offerings.
• Flexible to travel in-region and locally, as needed.
• Accommodate field observations and/or “ridealong” experiences, as requested.
