Clinical Team Manager

Responsibilities

• Assigns new client admissions to RN Case Managers and oversees clinical self-scheduling processes ensuring equal distribution of client assignments and scheduling.
• Reviews staff documentation, including OASIS to ensure compliance.
• Performs one-on-one case conference reviews and mentors staff appropriately.
• Affords assistance to field clinicians as necessary.
• Affords assistance with quality review as needed.
• Supervises, trains, assures adequate staffing levels and evaluates performance.
• Monitors quality of care provided and completion of charting.
• Holds staff accountable to goals.

Requirements

• Licensed Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) in Michigan.
• At least 2 years of home health experience.
• Prior nurse management experience is valued.
