About the role

We are now hiring a Clinical Team Facilitator (LPN/LVN) to join our dedicated home health team. This role supports patient care delivery by coordinating services, managing communications, ensuring accurate documentation, and collaborating with clinical staff and physicians.

Responsibilities

  • Carefully coordinate home health services, labs, equipment, and supplies according to physician orders and client needs.
  • Effectively manage incoming and outgoing calls, ensuring timely and accurate communication with providers, staff, patients, and families.
  • Accurately document physician orders, care plan updates, and all coordination activities in the EMR.
  • Support schedulers and care managers in arranging and adjusting patient visits based on changing needs.
  • Follow up promptly on lab results, medication changes, and physician communications.
  • Collaborate on process improvement initiatives to enhance patient safety, quality, and operational efficiency.
  • Tranquilly handle operational and clinical calls, maintaining calm and professional communication even in urgent situations.
  • Provide coordination during non-business hours as assigned.
  • Upkeep organizational values, ethical standards, and continuous quality improvement practices.

Requirements

  • Graduate of an accredited LPN/LVN program.
  • Active LPN/LVN license in the state of practice.
  • Two or more years of experience in home health, acute care, or ambulatory care.
  • Experience using electronic medical records and office software.
  • Valid driver's license and reliable transportation.
  • Strong verbal and written communication skills.
  • (California only): One to three years of professional nursing experience required by regulation.
  • Frequent standing, lifting, and walking.
  • Occasional patient positioning and lifting.
  • Adaptability to shifting priorities and potential on-call hours.

Qualifications

Minimum qualifications include:

  • One to three years of customer service experience (California only).
  • One year of professional nursing experience required by regulation.
