Clinical Social Worker - Substance Use Recovery (F/T Days Hybrid)

About the role

The Clinical Social Worker engages with patients across all phases of recovery as part of the Virtual Substance Use Recovery Program. This role requires advanced clinical expertise, including triaging initially stabilized patients, completing comprehensive assessments, establishing diagnoses, and delivering brief intervention-based therapy. The position functions with a high degree of autonomy while collaborating with an interdisciplinary care team.

Responsibilities

• Provide intensive case management services
• Address social determinants of health
• Coordinate care across providers and settings
• Connect patients with community and internal resources
• Perform comprehensive assessments and establish diagnoses
• Deliver brief intervention-based therapy using evidence-based practices such as motivational interviewing
• Develop and evaluate treatment plans and outcomes
• Collaborate with an interdisciplinary care team
• Other duties as assigned

Requirements

• Master’s degree in Social Work (required)
• Current LCSW license in the state of employment (required)
• Minimum 2 or more years’ experience in substance use, behavioral health, or acute hospital care setting (preferred)
