Job Summary and Responsibilities

Join Acentra Health as a Clinical Social Worker and play a vital role in strengthening the health and independence of the communities we serve. In this impactful position, you will conduct comprehensive, needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services delivered in private homes, adult care settings, or supervised living environments. As a trusted clinical professional, you will evaluate level-of-care (LOC) criteria to support informed, person-centered determinations that enable individuals to remain in or safely transition back to home and community-based settings. Your clinical judgment and compassionate approach will directly enhance quality of life and promote independence for priority populations.

This full-time, remote position supporting the state of Maine offers the opportunity to create meaningful impact within a collaborative, mission-focused organization.

Qualifications

  • Active and unrestricted Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) license for the state of Maine.
  • Graduate from an accredited Master's Degree Program.
  • Minimum of two years' experience in behavioral/mental healthcare.
  • Minimum of one year of assessment experience.
  • Strong clinical assessment and critical thinking skills.
  • Medical record abstracting skills.

Preferred Qualifications

  • Knowledge of the organization of medical records and medical terminology.
  • Excellent verbal and written communication skills.
  • Able to work in a team environment.
  • Flexibility and strong organizational skills.
  • Strong working knowledge of Microsoft Office Suite, including Word, Excel, Outlook, and Teams.

Pay

The pay range for this position is USD $67,300.00 - USD $80,000.00 /Yr.
