Clinical Social Worker - Broward County (Weekends)

About the role

A compassionate and patient-focused Clinical Social Worker is needed to join Trustbridge Hospice, a member of Empath Health, serving the Broward County area. This full-time position works weekends: Friday-Sunday, 8 a.m. – 8 p.m (36hrs).

Responsibilities

• Provide psychosocial assessment and intervention for patients and families as part of the interdisciplinary team.
• Perform duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family.

Requirements

• Degree in Social Work or a related degree such as Psychology.
• Masters preferred.
• One year of experience as a Social Worker. In a hospice setting is preferred.
• Ability to work effectively and flexibly as part of a collaborative interdisciplinary team.
• Strong written and oral communication skills.
