Clinical Social Worker

Role Overview: The Medical Social Worker collaborates with the interdisciplinary care team to assess and address patients' social and emotional needs in a home health setting. They develop personalized care plans, evaluate patient relationships and resources, and coordinate community support to promote recovery. Key Responsibilities Assess patients' social, emotional, and financial situations related to their health condition.Identify and facilitate access to community resources to support patient recovery.Communicate effectively with patients, families, and healthcare team members.Participate in care coordination and act as a resource within the team.Supervise and evaluate the Social Worker Assistant to ensure quality care.Maintain accurate documentation of patient interactions and care plans. Qualifications & Skills At least 3 years of social work experience in healthcare, home health, or hospice.Master's degree in social work and appropriate licensure.Valid driver's license, auto insurance, and CPR certification.Experience with discharge planning and community resource management.Strong organizational skills, ability to prioritize, and flexibility for local travel.Commitment to ongoing education and licensure maintenance.
