Schedule
8:30 AM - 5:00 PM alternating weekends
Responsibilities
- Support high-quality patient care and outcomes through case management practices
- Proactively identify patients at risk and initiate interventions within 24 hours of admission
- Conduct comprehensive psychosocial assessments and provide counseling, guidance, advocacy, and referrals
- Collaborate with interdisciplinary teams to develop and implement discharge plans
- Coordinate referrals to support services and educate patients and families about community resources
- Maintain accurate discharge planning, social services, and case management documentation
- Initiate and maintain acute-care abstracts and MedQM data
- Promote efficient resource utilization and support patients in achieving clinical and psychosocial outcomes
Requirements
- MS Social Work from an accredited school of social work
- Licensed by the Maryland Board of Social Work Examiners at the LMSW level, but LCSW-C is preferred
- 2 years or more of licensed clinical social work experience in an inpatient Hospital healthcare setting
- Aidin Discharge software experience is strongly preferred
Qualifications
- Must be licensed by the Maryland Board of Social Work Examiners at the LMSW level, but LCSW-C is preferred
Skills
Knowledge of case management practices, hospital policies, departmental procedures, and healthcare regulations
