Clinical Services Manager

About the Role

Provide community health nursing services in patients' homes, delivering comprehensive care and supporting health management. Work primarily in patient homes and office settings, maintaining a focus on community health improvement.

Responsibilities

• Deliver comprehensive nursing care in patients' homes.
• Supervise and collaborate with a multidisciplinary team including RN Case Managers, Visit RNs, LPNs, PTs, OTs, RDs, SLPs, MSWs, and Clinical Schedulers.
• Ensure adherence to HIPAA guidelines with Level 4 access to patient information.

Requirements

• BSN and at least two years of home care experience as a community health nurse or equivalent combination of education and experience.
• Active license and current registration to practice as a registered nurse in New York State.
