Clinical RN Case Manager

Responsibilities
- Assess patient needs and develop individualized care plans.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and families on health management and resources.
- Facilitate discharge planning and transitions of care.
- Maintain accurate medical records and documentation.
- Ensure compliance with HIPAA regulations and standards of practice.
- Advocate for patients’ needs within the healthcare system.
Qualifications
- Active RN license in the state of practice.
- Proven experience in case management or similar roles.
- Strong assessment and critical thinking skills.
- Knowledge of managed care principles and discharge planning processes.
- Excellent communication and interpersonal skills.
- Familiarity with medical terminology and documentation standards.
- Ability to work collaboratively in a team environment.
