Clinical Rehab Liaison
Job Summary
The Clinical Rehab Liaison at TriStar Health is a role that focuses on identifying inpatient rehab candidates and facilitating their transition from acute to rehabilitation settings. This position requires a strong background in clinical care, particularly in areas like nursing, speech therapy, physical therapy, occupational therapy, or respiratory therapy.
Qualifications
- Associate’s Degree in Nursing or Respiratory Therapy or Bachelor’s Degree in Physical or Occupational Therapy or Master’s Degree in Speech Therapy required.
- Current licensure as a Registered Nurse, Speech Therapist, Physical Therapist, Respiratory Therapist, or Occupational Therapist required.
- 2+ years of clinical care experience required.
- 2+ years of recent inpatient rehabilitation experience highly preferred.
- Experience as a clinical evaluator preferred.
- Certified Registered Rehab Nurse (CRRN) preferred.
- Basic Life Support (BLS) required.
What you will do in this role
You will assist with and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe. You will perform on-site and off-site clinical evaluations to assist the Rehab Medical Director in establishing patient eligibility for admission. You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit. You will obtain information relating to patient resources and healthcare insurance benefits.
