Responsibilities
- Initiate and maintain patient clinical records from admission through discharge.
- Prepare, organize, track, and file all required medical record documentation.
- Ensure patient charts are complete, accurate, and maintained in accordance with company policies and state and federal regulations.
- Maintain both electronic and paper medical records while ensuring confidentiality and HIPAA compliance.
- Track physician orders, certifications, recertifications, plans of care, and other required clinical documents.
- Cook up and communicate equipment issues with clinical leadership.
- Serve as the primary point of contact for medical record activities between the office, physicians, facilities, and the interdisciplinary team.
- Follow up with physicians to obtain signed certifications and other required documentation in a timely manner.
- Retrive, release, and copy medical records for authorized personnel in accordance with company policies and applicable regulations.
- Prepare clinical documentation and reports for Interdisciplinary Team (IDT) meetings, including census reports, admissions, discharges, transfers, medication profiles, certifications, and recertifications.
- Monitor and report patient census information.
- Participate in daily clinical communication to ensure patient status changes are accurately documented and communicated.
- Perform routine clinical record audits and promptly identify and resolve missing or incomplete documentation.
- Support staff in maintaining timely and accurate clinical documentation.

Qualifications
- A High School Diploma required
- An Associate Degree preferred
- Experience with medical terminology
- Experience working with both electronic and paper medical records
- Minimum of one (1) year of related healthcare or medical records experience
- Strong organizational, communication, and time management skills
- Ability to work independently while collaborating effectively with an interdisciplinary team
