About the role

Central Vermont Home Health & Hospice is a community-focused, not-for-profit organization dedicated to enhancing the well-being of central Vermonters through exceptional home health and hospice care. We aim to ensure access to high-quality services regardless of financial circumstances and offer a range of health promotion, long-term care, and maternal-and-child health services.

Responsibilities

  • Review and verify patient charts, OASIS assessments, plans of care, and other clinical documentation for completeness, accuracy, timeliness, and adherence to best practices.
  • Identify and resolve documentation discrepancies before they lead to survey deficiencies, claim denials, or negative patient outcomes.
  • Conduct audits of medical records and care processes, identify trends, and develop quality improvement initiatives to educate clinical staff.
  • Verify alignment between physician orders, the Plan of Care (485), and OASIS/clinical documentation.
  • Ensure compliance with Medicare, Medicaid, CLIA, OSHA, HIPAA, and Vermont Home Care Rules.
  • Support the agency's QAPI program, including data collection, trending, and performance improvement planning.
  • Compile and analyze quality indicators and performance metrics to recommend corrective actions, including those tied to PDGM and HHVBP reimbursement accuracy.
  • Collaborate with nurses, therapists, and social workers to ensure documentation reflects actual patient care and participate in QA meetings and interdisciplinary committees.

Qualifications

  • Ace of Associate's degree in nursing required; BSN preferred.
  • 2–3 years of experience in home health or hospice quality assurance or clinical documentation, with direct clinical experience in the specialty.
  • Valid and active Vermont Licensure.
  • Valid driver's license.
