Clinical Pharmacy Specialist – Solid Organ Transplant
About the role
The Clinical Pharmacy Specialist – Solid Organ Transplant is a decentralized clinical position that supports the continued growth and development of WVU Medicine’s existing kidney and heart transplant programs, as well as the implementation and expansion of the liver transplant program. This role provides comprehensive pharmaceutical care to adult solid organ transplant recipients across the continuum of care, including pre-transplant evaluation, peri-operative management, inpatient monitoring, discharge coordination, and longitudinal outpatient transplant clinic follow-up. The pharmacist will serve as the pharmacology expert on the interdisciplinary transplant team, optimize complex immunosuppressive and prophylactic regimens, manage high-risk drug interactions, and ensure safe and effective medication use in a high-acuity, highly regulated environment.
Responsibilities
- Maintains professional competence by attending and presenting at conferences, continuing educational programs, reading current professional literature and publishing clinical or practice-related experience or research.
- Provides leadership and care for patients in assigned areas of responsibility; guides providers and patients on the best practices and established guidelines for care and assures safe and effective use of drugs through clinical involvement.
- Monitors drug therapy of patients by participating in medical rounds or routinely evaluating the patient’s medication regimen, medical problems, laboratory data, and special procedures.
- Assists nursing and other disciplines or departments in admission and discharge planning and patient education related to the appropriate use of medication.
- Dispenses medications from the provider’s order. Ensures that policies and procedures related to drug distribution are followed.
- Reviews each order to verify and confirm accuracy and appropriateness. Evaluates compliance with established medication safety procedures.
- Communicates relevant findings and recommendations and documents them in the clinical information system.

Requirements
- Doctor of Pharmacy (Pharm.D) from an ACPE accredited school of pharmacy.
- Experience in at least ONE of the following areas: Completion of a PGY-2 Residency in an applicable clinical field, Completion of a PGY-1 Residency AND Board Certified in an applicable clinical field, Completion of a PGY-1 Residency AND three years of relevant pharmacist experience, Three (3) years of relevant pharmacist experience AND Board Certified in an applicable clinical field.
Qualifications
- Minimum Qualifications: Current licensure as required by state board where services will be provided: WV: Pharmacist license through the West Virginia State Board of Pharmacy, PA: Pharmacist license through the Pennsylvania State Board of Pharmacy, MD: Pharmacist license through the Maryland Board of Pharmacy, OH: Pharmacist license through the Ohio Board of Pharmacy. Obtain certification in Basic Life Support within 60 days of hire date.

Additional Job Description
This position is a Monday–Friday day shift role with no weekend, holiday, or call responsibilities and is fully on-site. Additional responsibilities include precepting pharmacy students and residents (PGY1 and PGY2 as applicable), contributing to transplant program development and workflow design—particularly in support of liver transplant services—participating in interdisciplinary committees, and engaging in research, quality improvement, and scholarly activities. Strong clinical expertise in transplant pharmacotherapy, excellent communication and collaboration skills, and the ability to support program growth while maintaining regulatory compliance are essential for success.
