Responsibilities
- Provides pharmaceutical care services, training and support to the staff and physicians of UF Health Central Florida focused glycemic management and HCHAP Medication Scores.
- Is responsible for glycemic management including the optimal use of hyperglycemic medications and improving medication-related HCHAPS scores.
- Is involved in various activities that include, but not limited to:
- Participation on Quality Improvement Teams
- Focus on Core Quality Measures
- Drug Use Evaluations
- Absence and involvement in varied medical staff committees/meetings;
- Adverse drug reaction reporting and follow-up;
- Formulary maintenance;
- One on one physician engagement.
Qualifications
- Education: Doctor of Pharmacy (Pharm.D.)
- Post Graduate Training: PGY1 or equivalent experience as clinical pharmacist
- PGY2 in specialty or equivalent experience
- Licensure/Certification/Registration: Valid Florida Pharmacist License
- Maintain Board Certification in specialty from the Board of Pharmacy Specialties (BPS) or equivalent certification if not offered by BPS
Special Skills/Qualifications/Additional Training/Experience Required
- Must be able to read, write, speak and understand English.
- Attend at least one nationally recognized meeting in area of specialty each year.
- Maintain current membership in organization dedicated to area of specialty.
