Clinical Pharmacy Specialist | Pharmacy | FT Days

Responsibilities

• Provides pharmaceutical care services, training and support to the staff and physicians of UF Health Central Florida focused glycemic management and HCHAP Medication Scores.
• Is responsible for glycemic management including the optimal use of hyperglycemic medications and improving medication-related HCHAPS scores.
• Is involved in various activities that include, but not limited to:
• Participation on Quality Improvement Teams
• Focus on Core Quality Measures
• Drug Use Evaluations

• Absence and involvement in varied medical staff committees/meetings;
• Adverse drug reaction reporting and follow-up;
• Formulary maintenance;
• One on one physician engagement.

Qualifications

• Education: Doctor of Pharmacy (Pharm.D.)
• Post Graduate Training: PGY1 or equivalent experience as clinical pharmacist
• PGY2 in specialty or equivalent experience
• Licensure/Certification/Registration: Valid Florida Pharmacist License
• Maintain Board Certification in specialty from the Board of Pharmacy Specialties (BPS) or equivalent certification if not offered by BPS

Special Skills/Qualifications/Additional Training/Experience Required

• Must be able to read, write, speak and understand English.
• Attend at least one nationally recognized meeting in area of specialty each year.
• Maintain current membership in organization dedicated to area of specialty.
