About the role
The pharmacist will have involvement with epilepsy, behavioral neurology, and movement disorders. The scope of work includes evaluating, optimizing, and counseling on medication regimens for mutual patients, providing chronic disease and medication therapy management, and working with providers to reduce the costs of medications.
Responsibilities
- Evaluate, optimize, and counsel on medication regimens for mutual patients
- Provide Chronic Disease and Medication Therapy Management directly to patients within a clinic setting
- Assist patients with cost, adherence, and lifestyle barriers related to their medications
- Develop prescribing, formulary, and clinical guidance for costly specialty medications
- Work closely with clinic staff to inform them of guideline and medication updates
- Collaborate with other care team members to develop care plans
- Navigate EPIC and utilize features within the EMR daily
- Coordinate educational programs and activities designed to enhance clinical drug knowledge of staff pharmacists, physicians, nurses, other care providers
- Works with staff pharmacist to increase their participation in drug monitoring, pharmacokinetics, and cost containment activities
- Provides pharmaceutical care to patients in assigned area of practice, implement cost containment programs, provide drug information, education, and clinical services which enhance the effectiveness of drug therapy and minimize patient risk
- Works with multiple medication automation systems
- Under a Collaborative Practice Agreement with the Neurology providers, manages patients’ neurologic chronic disease by prescribing, de-prescribing, adjusting doses, ordering, and monitoring labs
- Optimizes regimens and educates patients on Disease modifying therapies (DMTs)
- Completes comprehensive medication reviews and works with providers to reduce the costs of medications for their patients and the system, in addition to addressing any barriers to care to help improve adherence
- Be available for any pharmacy/medication related questions for both your patients and the team
- Actively participate in any team-based care huddles and provide recommendations to assist in constructing a care plan for patients
- Review data to assist in identifying patient populations needing pharmacy interventions
- Also, highlights data on pharmacy related metrics
- Be available for questions from providers in clinic or actively participate in provider’s visit if needed/requested
- Provide education to the inter-disciplinary team on pharmacy related topics, updates on guidelines, or current events on new medications/indications as requested
- Precept pharmacy students and pharmacy residents on site
- Evaluates patients and orders, and takes a lead role in the ordering and interpretation of drug serum levels, using pharmacokinetic principles
- Provides physicians with dosage regimen recommendations to enhance therapeutic outcomes, reduce patient’s risk, and reduce the cost of drug therapy
- Provides drug information and educational services to hospital health care professionals, patients and families and assists staff pharmacists in responding to drug information requests
- Serves as an information resource and clinical practice role model for staff pharmacists
- Coordinates and implements departmental drug utilization review efforts and other process improvement activities as assigned
- Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility
- Facilitates applicable Pharmacy Committee activities in area of specialty and participates on other committees / task forces as member, consultant and/or chair as assigned
- Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities
- Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis
- Coordinates the development, editing, and dissemination of departmental publications for physicians and hospital staff
- Precepts pharmacy residents and students as assigned
Requirements
- Bachelor's Degree in Science of Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
- 2 years of relevant experience pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency
- Preferred: 2 years of relevant experience hospital pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency
- Licensure - Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
- Licensure - Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
- Required: CRT-Basic Life Support (BLS) - AHA American Heart Association Ambulatory Sites 90 Days required
- Preferred: CRT-Basic Life Support (BLS) - ARC American Red Cross Ambulatory Sites 90 Days required
- Preferred: CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days preferred
- Preferred: CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days preferred
- Preferred: CRT-Board Cert Pharmacotherapy Specialist (BC-PS) - ASHP American Society of Health-Systems Pharmacists Upon Hire preferred
- Preferred: CRT-Board Cert Pharmacotherapy Specialist (BC-PS) - BPS Board of Pharmacy Specialties Upon Hire preferred
- Ideal candidates will possess a Doctor of Pharmacy degree and a PGY1 or PGY2 Residency in Ambulatory Care and/or Neurology
- Ideal candidates will possess Ambulatory Care Pharmacy experience or Neurology experience
- This is a Full Time position with shifts typically scheduled Monday to Friday, 8 a.m. to 4:30 p.m., No Weekends
- This is a Hybrid Position with the potential to partially work from home as agreed with the Practice Manager
