About the role

Sharp Memorial Hospital is seeking a Clinical Pharmacist Specialist-Critical Care to join their team. This role will be full-time and operates on a day shift.

Responsibilities

  • Provides clinical advisory services, demonstrating pro-active involvement and following through on advisory service outcomes.
  • Participates in clinical intervention programs, identifying and resolving prescribing, formulary, and therapeutic issues.
  • Completes and submits electronic forms consistently and responds to drug information requests promptly and professionally.
  • Reviews patient medication profiles for appropriateness and integrates patient variables, clinical status, drug knowledge, and professional judgment.
  • Communicates with nurses and physicians regarding therapeutic issues and follows up on formulary compliance issues.
  • Participates in Code-Blue emergencies, mixes and labels medications, and ensures pharmacy code-blue billing is completed where appropriate.
  • Engages in financial pillar/cost savings measures, such as IV to PO conversions and formulary adherence.
  • Performs clinical consult services, demonstrating competency in performing formal clinical consults and patient monitoring practices.
  • Administers medication to medical and surgical ICUs, ensuring accurate and appropriate medication orders, and managing critical care drug therapy.
  • Maintains multidisciplinary communication, clearly communicating goals and instructions to staff and fostering positive relationships with external customers.
  • Operates smoothly by providing daily work direction and supervision of technical and ancillary staff, overseeing personnel, and utilizing automation and technology.
  • Distributes medications accurately and in a timely manner, adhering to recognized standards of practice, legal and regulatory requirements, and hospital policies.
  • Ensures compliance with regulatory agency requirements, completing inspections, forwarding orders/examples to management, and ensuring medication storage security.

Requirements

  • California Pharmacist License from the CA State Board of Pharmacy.
  • AHA Basic Life Support for Healthcare Professional (BLS).
  • American Heart Association ACLS Certification.
  • Completed a Pharmacy Practice Residency with experience in critical care or a specialized clinical residency in critical care/emergency care (preferred).
  • Pediatric Advanced Life Support (PALS) certification (preferred).
  • BPS Board Certified Pharmacotherapy Specialist (BCPS) certification (preferred).
  • Current license to practice pharmacy in the state of California without restriction or probation.

Qualifications

  • Knowledge of chemotherapy dosing, monitoring, and established guidelines.
  • Ability to respond to and document drug information requests promptly and professionally.
  • Experience in critical care or specialized clinical residencies in critical care/emergency care.
  • Proficiency with and utilization of automation and technology in pharmacy operations.
  • Knowledge of age-specific medication delivery systems and IV admixture/aesthetic technique.
  • Compliance with regulatory agency requirements, including completing inspections and forwarding orders/examples to management.
