Clinical Pharmacist, Solid Organ Transplant

About the Role

The Pediatric Cardiac and Pulmonary Transplant Clinical Pharmacist will serve as an integral member of a busy, multidisciplinary heart and lung transplant program at Boston Children’s Hospital, providing comprehensive pharmacy services throughout the transplant continuum.

Responsibilities

• Completion of a chart/med review for all heart/lung transplant patients daily.
• Attendance at both Cardiac and Pulmonary multidisciplinary transplant rounds at a minimum of 2 days per week.
• Participation in transplant patient selection team meetings.
• Participation in transplant evaluations, including meeting with patients/families and documenting in the medical record.
• Documentation of Admission Notes in EMR for patients coming in for transplant surgery, with communication of plans to the clinical team.
• Provide pharmacy consultation and drug information as a subject matter expert for all solid organ transplant (SOT) teams and transplant nursing staff.
• Availability on weekdays for interpretation and evaluation of medication orders to ensure drug and dosage appropriateness and to identify possible adverse reactions, contacting physicians as needed.
• Optimization of immunosuppressive regimens, including induction, maintenance, and treatment of rejection.
• Completion of medication history documentation.
• Participation in quality improvement activities.
• Completion of discharge education, including medication teaching with patient/family and documentation in EMR.
• Ensure compliance with CMS and other applicable transplant regulatory and quality requirements, including maintaining documentation, monitoring program metrics, and supporting regulatory review readiness.

Requirements

• Knowledge of theories, principles, and concepts typically acquired through completion of a Bachelor's degree in Pharmacy plus up to one year of post-degree coursework and/or an internship.
• Two to three years of general pharmacy experience, or one year of directly related pediatric experience.
• Doctorate degree in Pharmacy preferred.
• Current license and registration by the Massachusetts Board of Registration in Pharmacy.
• Analytical skills to collect information from diverse sources, apply professional principles in performing analyses, and summarize data to solve problems or design systems/programs.
• Strong communication skills to effectively deal with conflicting views or issues, mediate fair solutions, and demonstrate well-developed writing skills.

Schedule

Days, with every 4th weekend.

Pay

The posted pay range reflects Boston Children’s reasonable and good-faith expectation for this role at the time of posting. Any base pay offer depends on skills, experience, education, certifications, and other job-related factors.
