About the role
The Pharmacist reviews all medication orders received from Physicians and Physician Extenders and dispenses medications after ensuring that drug, dose, frequency, and route of administration are appropriate. Reviews patient allergies, past medical history, drug interactions, and lab information for any potential clinical conflicts prior to dispensing medications. Responsible for inventory control and provides drug information support for other healthcare team members.
The Pharmacist oversees the work of all pharmacy students and technicians in the preparation of medications to ensure accuracy and safety in the medication use process. Collaborates with the Health Care Team on planning for continuum of care and discharge planning.
Responsibilities
- Review medication orders for appropriateness of drug, dose, frequency, and route of administration.
- Assess patient allergies, past medical history, drug interactions, and lab information for potential clinical conflicts.
- Dispense medications and manage inventory control.
- Provide drug information support to healthcare team members.
- Oversee pharmacy students and technicians to ensure accuracy and safety in medication preparation.
- Collaborate with the healthcare team on continuum of care and discharge planning.
Requirements
- Bachelor's Degree from a College of Pharmacy accredited by the American Council on Pharmaceutical Education Required.
- Doctorate Degree (PharmD) Preferred.
- 1 year of experience working in a hospital or Advanced Pharmacy Practice Experience (APPE) in a clinical setting Preferred.
- PA Pharmacist License Required.
