Clinical Pharmacist (Per Diem)

Under the leadership of the department Director, Assistant Directors, and/or Pharmacy Supervisors, the Clinical Pharmacist is responsible and accountable for planning, directing, assessing, and providing pharmaceutical care according to the policies and procedures of the hospital and department. Key dimensions include quality of care, professional commitment, teamwork, interpersonal skills, safety, customer service, fiscal responsibility, and a broad scope of pharmacy knowledge.

Responsibilities

  • Manage pharmacy protocols
  • Provide inpatient medication order screening and verification
  • Conduct patient consultations
  • Complete job functions and documentation in a timely manner
  • Perform patient assessment and medication management, including order processing and sterile compounding
  • Review patient medication regimens and perform therapeutic drug monitoring
  • Competently prescribe per drug protocols
  • Provide drug information and prevent, identify, and report adverse drug reactions and medication errors
  • Give patient consultations and develop plans of care
  • Oversee Pharmacy Technicians
  • Respond to various emergency situations, including codes
  • Cross-cover the Inpatient Pharmacist when needed
  • Engage in ongoing professional development, including attending continuing education conferences, staying current with pharmacy literature, and presenting patient cases or medication reviews

Requirements

  • At least one of the following:
    • Completed an accredited PGY1 Pharmacy Residency (preferred)
    • At least three years of experience in a Clinical Pharmacist position within the prior 5 years
    • At least five years of experience as an Inpatient Pharmacist or Acute-care Pharmacist
    • At least one year of experience in a Clinical Pharmacist position when applying for a night shift clinical pharmacist position
  • Education as required to meet State of California licensure requirements
  • Must pass orientation and initial competency assessment prior to independent assignment
  • Must complete and pass all annual competencies

Qualifications

  • State of California RPH (Pharmacist) license is required
  • American Heart Association Healthcare Provider BLS certification is required
  • ACLS certification must be completed prior to the end of the introductory period and independent assignment
  • Board certification preferred
  • DEA license required and must be completed at the end of the introductory period

Pay

Pay range: $97.11 to $129.87 per hour, based on years of applicable experience.

  • Hourly Evening Shift Differential: $5.80
  • Hourly Night Shift Differential: $10.00

Schedule

Assigned work hours are variable, 8-hour shifts. Position type: Per Diem.
