Clinical Pharmacist - per diem
About the Role
Under the direction of the Central Team Leader and Manager for Clinical Services, ensures optimum patient outcomes related to medication use by preventing or resolving drug-related problems and coordinating the acquisition, distribution, and control of all drug therapy to patients under the care of the pharmacist.

Responsibilities
- Ensures all drugs and medications are handled in the manner established by the Pharmacy and Therapeutics (P & T) Committee.
- Monitors drug therapy by direct patient contact, chart review, and reviews applicable laboratory data to identify or prevent drug-related problems or toxicity.
- Evaluates clinical indication for prescribed drugs, appropriate dose and route of administration, absence of therapeutic duplication, allergy history, and formulary compliance.
- Assesses pharmacokinetics, drug interactions, and nutrient interactions.
- Collaborates with Clinical Specialists, Physicians, and other medical staff on drug therapies and regimens.
- Reviews and approves medication orders, participates in daily rounds with the medical team, and enters physicians’ orders into the inpatient pharmacy system.
- Provides timely and accurate drug information to patients, nursing, and medical staff.
- Adjusts dosages based on renal function, age, and weight of the patient.
- Conducts interventions on IV to PO conversion and therapeutic interchange as needed.
- Ensures accurate and timely acquisition, distribution, and control of all medications, including investigational drugs.
- Maintains protocols and policies related to restricted drugs and narcotics.
- Ensures systematic, complete, timely, and documented assessment and intervention of patients.
- Supervises activities of supportive personnel, including student interns and technicians.
- Checks all medication and sterile products prior to dispensing and Pyxis refill deliveries.
- Assists in the orientation and training of pharmacy residents, students, technicians, and newly hired pharmacists.
- Conducts at least two in-service education programs for nurses or physicians annually.
- Develops and works on accomplishing annual goals in conjunction with the manager(s).
- Participates in the Adverse Drug Reaction (ADR) Reporting Program, Drug Use Evaluation (DUE) Program, and other quality assurance activities.
- Ensures compliance with federal, state, and local regulations, as well as standards from The Joint Commission.
- Resolves patient complaints and reports significant issues to the Patient Advocate or Director of Quality Improvement.
- Maintains professional competencies through continuing education, literature review, and professional meetings.
- Follows all departmental policies and procedures, including mandatory training and confidentiality standards.
- Performs other related tasks as needed.
Requirements
- PharmD degree (or Bachelor’s degree plus 2 years of hospital pharmacy experience).
- Licensure to practice pharmacy in Massachusetts or eligibility to become fully licensed within 45 days of hire.
- Prior hospital pharmacy experience preferred but not required.

Pay
Compensation range: $43.03 - $62.50 per hour. This range is based on minimum job qualifications and considers factors such as education, experience, skills, certifications, internal equity, and market competitiveness.
