Clinical Pharmacist - Part Time

About the Role

Option Care Health, Inc. is the largest independent home and alternate site infusion services provider in the United States. As a Clinical Pharmacist – PT, PD, CA, you will provide safe and appropriate pharmacy services to patients in accordance with company policies, professional standards, and regulatory requirements. This entry-level position offers the opportunity to grow your career in clinical pharmacy within the alternate site setting.

You will coordinate patient services with the Nursing Department, monitor patient progress, and educate patients, caregivers, and professionals on appropriate medication use. This role is integral to re-imagining the infusion care experience for patients and contributing to a culture of inclusivity, respect, and innovation.

Responsibilities

• Determine the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing their medical condition and prescribed medication therapy.
• Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for home care patients, including making modifications to prescribed medication therapy or monitoring parameters as needed.
• Complete nutritional assessments and make appropriate recommendations.
• Modify pharmacy care plans based on evaluation of monitoring data and other patient-specific information, demonstrating competency in pharmacokinetic principles and laboratory value assessments.
• Provide medication-use education to Option Care staff, other professionals, patients, and caregivers using effective patient education techniques.
• Ensure continuity of pharmaceutical care between the home and other patient-care settings through systematic communication of pertinent patient information.
• Prepare and dispense medications using appropriate techniques, ensuring proper concentration, compatibility, stability, and storage while adhering to safety, quality assurance, and regulatory practices.
• Comply with state/federal laws and USP standards, including DEA controlled substance regulations and perpetual inventory systems for schedule II narcotics and controlled substances.
• Manage the use, maintenance, and troubleshooting of medication administration equipment and related devices, including assessing, managing, and reporting medication-related incidents or equipment failures.
• Provide concise, applicable, and timely responses to drug information requests from healthcare providers and patients, based on literature analysis.
• Attend educational seminars or participate in continuing education opportunities to maintain professional competency and comply with state-mandated requirements.
• Manage the use of investigational drug products according to established regulations and company policies.
• Utilize pharmacy support personnel effectively by prioritizing workload, organizing workflow, and ensuring accuracy in their work.
• Maintain confidentiality of patient and proprietary information in accordance with legal and ethical guidelines.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following:
  • At least one year of experience in sterile IV admixture procedures, or
  • Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), or
  • Completion of an ASHP accredited Pharmacy Residency Program, or
  • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or
  • Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
• Access to a reliable means of transportation for home visits, including a valid driver’s license and proof of insurance if using a personal vehicle.
• Experience providing customer service to internal and external customers.
• Basic proficiency in Microsoft Office applications (Word, Excel, PowerPoint).
• Willingness to travel up to 5% of the time for business purposes (within state).

Preferred Qualifications

• Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
• Experience in home care, hospital, or other clean room settings.
• At least 1 year of experience in indirect management of team members, including assisting in development, training, and assignment of work/projects.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay range is $44.24–$73.73 per hour.
