About the role

The clinical pharmacist is responsible for providing pharmaceutical care services to clinic patients, including medication therapy management, drug therapy consultation, chronic disease state management, transitions of care, and patient education and counseling. The role involves participation in clinic and pharmacy staff development, patient care coordination meetings, and the clinical practice guideline review and development process.

Responsibilities

• Provide high-quality pharmaceutical care to ambulatory patients, including medication reconciliation, therapeutic selection, implementation, monitoring, modification, and dispensing of drug therapy.
• Deliver comprehensive patient education and counseling on medication therapy, including the use of administration devices (e.g., injection devices, inhalers).
• Collaborate with providers and healthcare professionals to individualize drug therapies based on patient parameters, current literature, safety, efficacy, and cost.
• Participate in medication therapy management, including monitoring and modifying drug therapy as outlined in collaborative drug therapy agreements (CDTA).
• Engage in chronic disease state monitoring using registries or databases to enhance therapeutic outcomes through education, adherence monitoring, and drug therapy optimization.
• Participate in interdisciplinary team activities to develop and implement clinical plans of care, disease state management programs, treatment protocols, and medication error reduction strategies.
• Provide pharmaceutical care oversight and consultation for high-risk patients, including those with 10+ medications, high resource utilization, advanced age, or on high-risk/specialty medications.
• Support transitions of care by collaborating with inpatient providers for complex patients and performing medication reconciliation for hospital discharge patients.
• Identify patient barriers to medication adherence, considering factors such as language, cultural/religious practices, physical, cognitive, emotional, and socioeconomic status.
• Document drug information questions, drug utilization evaluation activities, and adverse drug reactions.
• Participate in the development and documentation of quality assurance indicators, such as drug use evaluation.
• Maintain clinical competency through self-study modules and provision of clinical services.
• Provide teaching, direction, and supervision to pharmacy residents, technicians, interns, and students, as well as training for new pharmacists.
• Participate in projects aligned with the strategic goals and initiatives of the department and organization.
• Assist in the Pharmacy and Therapeutics Committee process for review and evaluation of treatment options and promote adherence to the VMC Drug Formulary.
• Conduct periodic reviews of prescribing practices and design targeted prescriber education to promote compliance with clinical practice guidelines.
• Actively participate in committees responsible for medication use, quality improvement, performance improvement, and other aspects of patient care.
• Maintain knowledge of outpatient medication reimbursement models, including MTM and pharmacist provider status.
• Develop and present periodic in-services to healthcare providers.
• Participate in research studies within the clinical service.
• Maintain competency in all applicable CDTAs and complete departmental and organizational competency assessments.
• Complete additional responsibilities/projects as assigned by supervisor/manager/director.

Requirements

• Graduate of an accredited College of Pharmacy; PharmD degree strongly preferred.
• Licensed as a Registered Pharmacist in the State of Washington with license in good standing, or eligible for licensure.
• Completion of an ASHP-accredited PGY1 residency required or equivalent practice experience.
• PGY2 residency preferred for specific practice areas (e.g., Oncology).
• American Heart Association’s Basic Life Support (BLS) for Healthcare Providers certification (active or obtained within 30 days of employment).
• Minimum of 2-3 years of clinical experience in a practice setting preferred.
• Board certification or equivalent advanced practice specialty highly desirable.

• Ability to pass oral and written examinations pertaining to VMC clinical pharmacy responsibilities.

Specialty Practice Setting Prerequisites

• Medication Management Clinic: PGY1 pharmacy practice residency required or demonstrated equivalent practice experience. Board certification preferred.
• Oncology Clinic/Infusion Center: PGY1 pharmacy practice residency required. PGY2 residency in Oncology required or demonstrated equivalent practice experience. Board Certified Oncology Pharmacist credential preferred.
• Primary/Specialty Care Clinics: PGY1 pharmacy practice residency required. PGY2 residency in Ambulatory Care required or demonstrated equivalent practice experience. Board Certified Ambulatory Care Pharmacist credential preferred. Certified Diabetes Educator credential desirable. Minimum of 1-2 years of clinical experience in a specialized practice setting preferred.
• Specialty Pharmacy: PGY1 pharmacy practice residency required or demonstrated equivalent practice experience. Minimum one year of specialty pharmacy practice experience preferred. Ability to participate in activities related to specialty pharmacy accreditation. Knowledge and ability to perform duties of a Retail Pharmacist with respect to processing specialty pharmacy prescriptions. Board certification preferred.

Pay

$77.78–$107.86/hour, DOE.
