Clinical Pharmacist- Outpatient

Summary

Position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Applicants must meet specific eligibility requirements per VHA policy and submit the EDRP application within four months of appointment. Award amounts (up to $200,000) and eligibility periods (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the application. Former EDRP participants are ineligible to apply.

Responsibilities

• Analyze real and potential drug‑related medical problems and implement corrective actions to ensure optimal drug therapy.
• Apply standards for distribution and control of scheduled and non‑scheduled drugs.
• Evaluate appropriateness of drug therapy based on patient‑specific factors and individualize therapy.
• Dispense medications, provide patient consultations, and deliver drug information.
• Support outpatient pharmacy operations, including prescription/order processing, outpatient dispensing, and other assigned pharmacy activities on any shift.
• Select drug therapy using pharmacokinetic and pharmacodynamic principles, monitor efficacy, side effects, and clinical outcomes, and advise prescribers.
• Provide patient‑specific therapeutic drug monitoring and communicate findings to health‑care providers orally or in writing.
• Maintain required records and serve as a drug‑information resource for prescribers, health‑care professionals, patients, and caregivers.
• Report adverse drug events, near misses, and medication errors in alignment with the VA ADERS reporting program.
• Manage medication orders, including flagging prescriptions, handling alerts, and generating unassigned discontinued orders using service reject.

Work Schedule

• Monday‑Friday, various 8‑hour shifts.
• Rotating weekends and holidays.
• Schedule subject to change based on agency needs.
