Clinical Pharmacist (NH-licensed)

Provide integrated support to patients and physicians by developing personalized relationships to improve medication adherence and promote evidence-based care.

Responsibilities

  • Review and interpret clinician’s patient care notes/assessments for clinical data needed for prior authorization approvals and care plan recommendations.
  • Develop and refine disease and drug-specific clinical protocols to maximize patient adherence and promote evidence-based, cost-effective care.
  • Monitor ongoing patient adherence (PDC reports, patient assessment data, and mobile platform data) across all sites and disease categories, with a focus on oncology and other specialty disease states.
  • Evaluate EMR to review drug and disease interactions, medication selection, labs/vitals, allergies, RX orders/renewals, and direct MD communication to optimize patient outcomes.
  • Recommend alternative therapeutic drug regimens and disease-specific treatments according to guidelines.
  • Monitor and evaluate therapies for side effects or medication-related issues; recommend drug therapy changes to prescribers as appropriate.
  • Perform medication therapy management (MTM), including reviewing and monitoring medication regimens, advising clinicians on evidence-based therapy, documenting interventions, and assessing patient adherence rates.
  • Conduct clinical interventions such as pharmacist consultations, motivational interviewing/coaching, drug regimen optimization, medication reconciliation, medication synchronization, coordination of home nurse visits, and transitions between liaison/PSC.
  • Oversee tech’s daily activities, including verification of delivery coordination slips, prior authorizations, PA log activity, and proactive management care utilization within each specialty.
  • Identify and resolve insurance-related issues; recognize opportunities for financial assistance.
  • Supervise communication of support personnel in compliance with HIPAA rules and regulations.
  • Maintain current registration and ensure all remote pharmacist activity complies with local, state, and federal drug laws as regulated by each state board of pharmacy and the DEA.
  • Communicate directly with other health professionals regarding patient care.
  • Develop personal knowledge by participating in education programs; train pharmacy staff, students, interns, externs, and healthcare professionals.
  • Manage, organize, and update relevant data using database applications.
  • Apply observations and recommendations to operational issues to improve productivity, quality, and customer-service standards.
  • Assist with research projects and peer-reviewing articles as needed.
  • Identify trends, resolve problems, recommend improvements, and implement change.
  • Actively participate in process improvement initiatives.
  • Protect organizational value by keeping information confidential.
  • Update job knowledge through educational opportunities, professional publications, personal networks, and professional organizations.
  • Perform other duties as assigned.

Requirements

  • Doctor of Pharmacy (PharmD) or equivalent advanced degree.
  • Licensed Pharmacist.
  • 1+ years of experience is strongly preferred but not required. If additional licensure is needed, the applicant must commit to obtaining it within a mutually agreed-upon timeframe.
  • Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint) for communication, documentation, and data management.
  • Strong computer skills, including navigating web-based platforms; data entry skills will be evaluated through a pre-employment assessment.
  • Knowledge of EPIC/QS1/WILLOW software is a plus.
