About the role

The clinical pharmacist at UMass Memorial Health provides comprehensive medication management services that integrate clinical services, order management, patient/staff education, and research. They are responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes.

Responsibilities

• Provides medication management services in a collaborative manner, which includes reviewing patient medication history, educating patients on drug therapy, establishing desired outcomes, developing/ modifying therapeutic plans, determining alternatives for achieving outcomes, developing drug monitoring plans, monitoring patient outcomes, documenting medication related issues in electronic health record, interpreting laboratory data, reviewing patient medication profiles, monitoring pharmacokinetics of drug therapy, monitoring for potential adverse drug – drug, drug-lab, drug-disease state, and drug - food interactions.
