Job Details / Responsibilities

• Provide clinical pharmacy services in an inpatient hospital setting
• Review and verify medication orders for appropriateness
• Collaborate with physicians, nurses, and interdisciplinary teams on patient care plans
• Monitor patient therapy for efficacy and safety, including pharmacokinetic dosing
• Participate in code responses and emergency situations as needed
• Education patients and staff on medication use and safety
• Support hospital formulary management and medication safety initiatives
• Work a mix of day and evening shifts with every other weekend rotation

Qualifications

• Doctor of Pharmacy or BS Pharmacy degree required
• Minimum of 2 years of pharmacist experience
• Hospital experience preferred
• Pharmacy state licensure required within 60 days of start date
• BLS or ACLS required
• PALS required for pediatric practice
