Responsibilities
- Determines patient suitability for home care by collecting and organizing patient-specific information.
- Assesses patients' medical conditions, prescribed medication therapies, and prognoses to ensure alignment with home care services.
- Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans.
- Modifies pharmacy care plans based on monitoring data and patient-specific information, using pharmacokinetic principles.
- Provides medication-use education to staff, patients, and caregivers.
- Ensures continuity of pharmaceutical care between home and other patient-care settings.
- Prepares and dispenses medications following safety and quality assurance practices.
- Manages and troubleshoots medication administration equipment.
- Responds to drug information requests from healthcare providers and patients.
- Manages investigational drug products according to regulations and company policies.
- Supervises and ensures accuracy of pharmacy support personnel’s work.
- Maintains confidentiality of patient and proprietary information.
Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license granted by the State Board of Pharmacy.
- One of the following:
- At least 1 year of experience in sterile IV admixture procedures.
- Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
- Completion of an ASHP accredited Pharmacy Residency Program.
- Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- Equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services.
Skills
- Experience providing customer service to internal and external customers.
- Basic proficiency in Microsoft Word, Excel, and PowerPoint.
Preferred Qualifications
- Completion of an ASHP accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience.
- Experience in home care, hospital, or clean room setting.
- At least 1 year of indirect management experience, including team development and training.
Travel Requirements
Willing to travel up to 5% of the time for business purposes (within state).
Pay
Salary range: $107,330.80 - $178,899.67, determined by education, experience, skills, and market data.
