Clinical Pharmacist

About the Role

The Clinical Pharmacist delivers telephonic Comprehensive Medication Management (CMM) services nationwide, helping participants with complex medication regimens and multiple chronic conditions optimize their medications and resolve therapy-related issues. This role combines clinical expertise with operational excellence, focusing on high-quality consultations, efficiency, and teamwork.

Responsibilities

• Provide telephonic CMM consultations balancing clinical quality, outcomes, efficiency, and productivity.
• Ensure clear recommendations and appropriate follow-up with participants and providers after each encounter.
• Document interactions accurately and consistently.
• Meet daily/weekly expectations for consultation volumes, documentation, and follow-up.
• Partner with Guides and team members to integrate medication management into participants’ broader care journeys.
• Use motivational interviewing and coaching to support self-management and adherence.
• Identify barriers to medication success and develop practical solutions with participants.
• Contribute ideas to improve workflows, processes, and team scalability.
• Stay current on clinical information and apply it to achieve outcome-driven results.
• Maintain required state licensures and serve as Pharmacist-in-Charge (PIC) if assigned.
• Represent MOBE with state Boards of Pharmacy during licensing/audits and ensure compliance with regulations.
• Provide compliance oversight for remote pharmacy advisory services in relevant states.
• Precept pharmacy students and interns.
• Perform other duties to fulfill MOBE’s mission and Guiding Principles.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited school.
• Licensed Pharmacist in any state in good standing (no disciplinary actions).
• Willingness to obtain MN license within 3–6 months of hire (register as Pharmacist-intern before start if not already licensed in MN).
• Willingness to obtain and maintain multi-state licensure and/or PIC status as needed.
• 2+ years providing CMM and/or MTM in a fully remote or hybrid setting.
• 5+ years as a Clinical Pharmacist, including outpatient settings.
• 3–5 years working with complex medication regimens and multiple chronic conditions.
• 2+ years of Supervisor/Lead experience (may provide compliance oversight for remote pharmacy services).

Preferred Qualifications

• Experience conducting telephonic CMM/MTM consults.
• Experience serving as Pharmacist-in-Charge (PIC).
• Bilingual preferred, not required.

Pay

The salary range for this position is $125,000–$140,000 based on experience and qualifications.
