Responsibilities
- Review clinician’s patient care notes for prior authorization and care plan recommendations
- Develop and optimize disease/drug-specific clinical protocols
- Monitor patient adherence using various data sources
- Evaluate EMR data to enhance medication adherence
- Communicate drug regimen changes and recommendations to prescribers
- Conduct medication therapy management (MTM)
- Participate in process improvement initiatives
Qualifications
- Doctor of Pharmacy (PharmD) or equivalent advanced degree required
- Licensed Pharmacist with valid registration
- 1+ years of pharmacy experience preferred
- Familiarity with specialty medications in oncology and chronic disease management
- Proficient in Microsoft Office and database applications (experience with EPIC/QS1/WILLOW software is a plus)

Pay
$122,000 – $128,000 annually
