Clinical Pharmacist

Responsibilities

• Review clinician’s patient care notes for prior authorization and care plan recommendations
• Develop and optimize disease/drug-specific clinical protocols
• Monitor patient adherence using various data sources
• Evaluate EMR data to enhance medication adherence
• Communicate drug regimen changes and recommendations to prescribers
• Conduct medication therapy management (MTM)
• Participate in process improvement initiatives

Qualifications

• Doctor of Pharmacy (PharmD) or equivalent advanced degree required
• Licensed Pharmacist with valid registration
• 1+ years of pharmacy experience preferred
• Familiarity with specialty medications in oncology and chronic disease management
• Proficient in Microsoft Office and database applications (experience with EPIC/QS1/WILLOW software is a plus)

Pay

$122,000 – $128,000 annually
