Clinical Pharmacist
About the role
This position is responsible for delivering appropriate, safe, efficient, and cost-effective pharmaceutical care within the scope of their assigned practice. They also oversee and participate in ensuring appropriate medications are prepared and delivered to care areas.
Responsibilities
- Reviews patients' medication orders for allergies, drug interactions, and appropriateness of therapy.
- Makes evidence-based interventions to optimize medication use.
- Safely prepares and delivers medications on time to care areas.
- Counsels patients and promotes quality medication education and adherence.
- Ensures patient information is clear and complete, and complies with regulatory requirements.
- Serves as a resource for other health professionals, providing medication-related clinical consultation and clarification.
- Solves problems related to equipment, maintenance, drug availability, staffing, and patient care.
- Identifies areas for improvement and potential safety hazards.
- Participates in departmental meetings and supports local and enterprise pharmacy leadership initiatives.
Requirements
Minimum qualifications include a Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution, current license to practice pharmacy in the state of practice, and knowledge of state and federal regulations.
Preferred qualifications include experience in solid organ transplant, completion of a PGY2 Solid Organ Transplant Pharmacy residency, or 3 years of transplant experience.
Qualifications
- Minimum Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).
- Licenses to practice pharmacy in the state of practice and dispense controlled substances.
- Effective communication, people management, and project management skills.
- Immunization certification if applicable to practice site needs.

Pay
The hourly range for this position is $62.89 - $92.44, with actual hourly rates dependent upon experience.
Schedule
This position is Monday through Friday from 7-3:30pm. Weekends and holidays are required, with one weekend about every 7 weeks and one major or minor holiday each year.
