The Clinical Pharmacist ensures optimal drug therapy for patients in their clinical specialty area and provides educational activities to the pharmacy staff and other caregivers to promote safe and cost-effective drug therapy.

Responsibilities

• Evaluates drug literature by analyzing experimental design and methodology to compare therapeutic regimens and roles for new drugs, serving as an authoritative resource on drug utilization to promote optimal, cost-effective drug therapy that maximizes clinical outcomes.
• Provides clinical support to staff pharmacists, collaborating directly with healthcare providers to resolve medication-related issues and offering patient-specific drug therapy information.
• Designs or develops therapeutic regimens or protocols based on patient-specific information to maintain the safety of drug therapy using various screening mechanisms.
• Assists in training staff, including serving as a preceptor for pharmacy residents and students, mentoring trainees, contributing to weekly consult rounds, and educating nursing staff, physicians, and other caregivers through formal and informal presentations.
• Participates in the Pharmacy clinical support team and multidisciplinary patient care committees, conducts drug use evaluations, documents medication interventions and adverse drug reactions, and engages in projects to enhance clinical pharmacy services.
• Contributes to system Pharmacy & Therapeutics (P&T) Council and applicable system councils in developing drug regimens, order sets, and protocols within areas of expertise.
• Participates in ongoing continuous improvement of department workflow via the lean daily management framework.
• Maintains skill and competency to staff in pharmacy, preferably in the area of expertise, as needed.

Requirements

• Current Connecticut Pharmacist license or eligible to receive licensure within 45 days of hire.
• Bachelor’s in Pharmacy required; PharmD preferred.
• Completion of an ASHP-approved pharmacy residency program or 3 to 5 years of pharmacy experience.
• Board Certified Pharmacotherapy Specialist (BCPS) preferred.
