Description

The Clinical Pharmacist in the Population Health and Prescription Management (PHARXM) provides clinical and educational functions to support patient care. This includes managing medication therapy for patients as outlined in medical staff-approved protocols, providing consults, developing individualized medication plans, documenting interactions, and assisting in transitions of care.

Professional Duties

  • Follow medical staff approved protocols in managing patients under PHARxM department.
  • Provide comprehensive medication therapy consults to patients referred to PHARxM.
  • Develop individualized medication plans to mitigate adverse side-effects.
  • Provide general drug information to Providers to support quality patient care.
  • Document patient interactions and consultations in the patient's EMR/EHR according to departmental policies, procedures, and timeframes.
  • Refer patients to other departments or programs for untreated health problems.
  • Conduct comprehensive medication histories, assess patient compliance, and recommend therapy modifications to optimize clinical outcomes.
  • Absorb and ensure safe, effective, and cost-efficient medication therapy for all patients.
  • Establish and maintain policies and procedures for drug usage, distribution, and control to ensure regulatory compliance and safe medication practices.
  • Perform other duties as assigned.
  • Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience.

Qualifications

  • PharmD, BS in Pharmacy, or foreign equivalent, with a valid CA RPh license required.
  • Current California Pharmacist License required.
  • 0-2 years of pharmacy experience required.
  • Active member of a State or National Pharmacy Organization, preferred.
  • Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications.
  • Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors, and others.
  • Ability to work independently and as part of a team.
  • Ability to manage time effectively and prioritize tasks to meet established deadlines.
  • BLS required within 60 days of hire. BLS not required for employees who are not involved in direct patient care.
  • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
