SUMMARY

Reports to Director Brown University Health Pharmacy, Director Specialty Pharmacy, or Ambulatory Manager as applicable. Responsible for interpreting, evaluating, preparing, and dispensing medications in compliance with established policies, procedures, standards of practice, and scope of pharmacy ambulatory services. Collaborates with multidisciplinary care providers to ensure safe, appropriate, and cost-effective medication use.

RESPONSIBILITIES

  • Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for patient populations.
  • Evaluates and prepares medications according to patient needs and ensures adherence to policies and regulations.
  • Communicates with prescribers to address medication concerns and optimize treatment plans.
  • Ensures accurate and comprehensive patient education on medication use.
  • Participates in quality improvement initiatives and collaborates with healthcare professionals.
  • Maintains and updates patient medication profiles and records.
  • Collaborates with specialty pharmacy and medication access program teams to manage patient care effectively.
  • Monitors and interprets laboratory results and adjusts medication regimens as needed.

MINIMUM QUALIFICATIONS

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
  • Possesses current unrestricted licensure as a licensed pharmacist in the State of Rhode Island.
  • Within 6 months of start date, obtain American Council for Pharmacy Education (ACPE) approved Immunization Certification and American Heart Association Basic Life Support (BLS) Certification for positions requiring immunizations or medication administrations.
  • Two to three years progressively more responsible related experience, preferably in a diverse and operationally complex health care environment.
  • Knowledge of pharmacy information systems, EHRs, automated dispensing technology, and personal computers.

EXPERIENCE

  • Experience in a similar health care environment preferred.
  • Knowledge of pharmacy information systems, EHRs, automated dispensing technology, and personal computers.
  • Occasional travel between facilities or sites as required.
  • Effective interpersonal and communication skills.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS

  • Extended periods of time spent standing and walking.
  • Requires visual and manual dexterity to operate a computer.

INDEPENDENT ACTION

Performs independently within department policies and practices. Refers specific complex problems to supervisor where clarification of departmental policies and procedures may be required.

$105,019.20-$173,264.00
