Qualifications
- A valid certification or degree as a licensed practical nurse (LPN) or licensed social worker (LSW).
- An active and unrestricted license in the relevant state for this position.
- Three-plus years of experience in hospital or post-acute care settings.
- Proficiency in technology, including electronic medical systems and Microsoft Office.
- The position requires a passion for improving patient outcomes and working in a collaborative team environment.
- Authorized to work in the United States.
Key Responsibilities
- Partner with other Sound nurses, site leaders, and the hospitalist team to improve the quality and efficiency of hospitalized patient care.
- Report to nursing leadership.
- Improve patient outcomes by promoting health, preventing readmission, and addressing barriers to healthcare access before discharge.
- Work to improve the patient experience and optimize resource utilization.
- No direct patient care.
- Interact directly with patients at the bedside through patient satisfaction and discharge preparedness conversations.
Scheduling
Enjoy flexible scheduling in a fast-paced environment with a Monday through Friday workweek from 7:00 AM to 4:00 PM (including a lunch break). No nights, no weekends, and paid holidays.
