Clinical Oncology Nurse Navigator-Cancer Center- Mount Sinai Queens- Per Diem Days (10 hrs)

About the Role

The Per Diem Clinical Oncology Nurse Navigator (ONN) is a professional registered nurse with oncology-specific clinical knowledge. This role provides individualized assistance to patients, families, and caregivers through informed decision-making, timely access to quality health and psychosocial care, and coordination of appropriate follow-up visits throughout the cancer continuum. The ONN helps overcome healthcare system barriers, influences systems and behaviors, and addresses gaps in care through research and evidence-based practices guided by regulatory committees and professional organizations.

The Per Diem Clinical Oncology Nurse Navigator supports organizational strategies to deliver high-quality care through community needs assessment, navigation program development, marketing and outreach, and ONN-specific outcome metrics (e.g., care coordination, guidance, education, and advocacy). This role also initiates, develops, and implements mechanisms for effective collaboration with the interdisciplinary team, including new patient coordinators, clinical oncology associates, clinical coordinators, infusion nurses, advanced practice providers, physicians, social workers, and nursing/administrative leadership.

Responsibilities

• Delivers patient care by incorporating the tenets of the professional practice model of Relationship-Centered Care and through the application of the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards.
• Organizes and leads day-to-day patient care coordination, as appropriate to program objectives and areas of clinical expertise.
• Assists patients in accessing cancer care and navigating healthcare systems using thorough chart reviews in EMR systems.
• Assesses barriers to care and engages patients and families in creating potential solutions to financial, practical, and social challenges.
• Identifies appropriate and credible resources responsive to patient needs (practical, social, physical, emotional, spiritual), considering reading level, health literacy, culture, language, and the amount of information desired.
• Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of a robust plan of care throughout the cancer care continuum.
• Establishes and implements short- and long-term goals, objectives, policies, and operating procedures to ensure high-level performance in nursing care delivery to the specialty patient population.
• Educates patients and caregivers on the multidisciplinary nature of cancer treatment, the roles of team members, health promotion, and what to expect from the health system, while anticipating needs and readiness to learn.
• Follows up with patients to support adherence to the agreed-upon treatment plan through continued non-clinical barrier assessment and referrals to supportive resources in collaboration with the interdisciplinary team.

Requirements

• Bachelor of Science with a major in nursing required.
• Master’s degree preferred.
• 3-5 years minimum demonstrated clinical and administrative expertise in the area of nursing practice assigned.
• 2-3 years’ experience in Oncology.
• Licensed as a registered nurse with current registration in New York State.
• ONS/ONCC Chemo Bio Certification.

Pay

The salary range for this role is $70 - $70 hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The range does not include bonuses, incentive pay, differential pay, or other forms of compensation or benefits.
