Clinical Nutritionist/Registered Dietitian

About the role

We are hiring a HYBRID Clinical Nutritionist (RDN/CDN) to join our Nutrition Department. As a Nutritionist, you will provide education and counseling to high‑risk patients and their families in our primary care and multi‑specialty health centers. This opening is stationed at our health center in Bronx, NY, with an emphasis on supporting high‑risk patients treated through medical specialists (e.g., Cardiology, GI, Endocrinology, Nephrology). The associate will deliver Medical Nutrition Therapy (MNT), education, and behavior‑change counseling to improve health outcomes related to chronic disease.

Responsibilities

• Assess symptoms, nutrition needs, vitals, labs, diagnoses, supplement use, allergies, and food access.
• Provide MNT, preventative care, education, and nutrition counseling to patients and their families.
• Supply appropriate educational materials, ensure health‑literate communication, and develop individualized plans.
• Schedule and conduct reassessments of patients to follow up on goals and behavior change.
• Provide initial and follow‑up consultations to patients.
• Engage in outreach and therapy for patients with unhealthy weight and chronic illnesses.
• Participate in food demonstrations, health fairs, and workshops in collaboration with a multi‑disciplinary team.
• Serve as a key member of the interdisciplinary team of providers caring for our patients.

Qualifications

• Bachelor’s or Master’s degree in Nutrition/Dietetics required.
• New York State Licensed CDN required; RDN on track for CDN welcomed to apply.
• Bilingual Spanish/English required.
• Passion for community health.

Pay

Minimum $60,000 per annum; Maximum $77,000 per annum.

Schedule

Hybrid work arrangement in Bronx, NY. Monday to Friday, 8:00 am – 5:00 pm. Three days onsite, two days remote.
