Responsibilities
- Facilitate the entry of patients into the cancer program and serve as the point of contact for primary care physicians and referring providers.
- Provide individualized assistance to Medical Oncology patients, families, and caregivers in the coordination and communication of the patient's plan of care.
- Initiate and document patient teaching related to diagnosis, treatment plans, symptom management, medications, and supportive care services.
- Perform telephone triage and assess patient concerns to ensure timely intervention and appropriate follow-up.
- Follow up on recommendations from Harris Health System's Ask My Nurse service.
- Implement risk assessments and healthcare pathways for oncology patients.
- Collaborate closely with the interdisciplinary team to coordinate and maintain individualized patient care plans.
- Develop collaborative relationships with all physicians in the UT MD Anderson Oncology Program at LBJ Hospital.
- Serve as a liaison between oncologists and patients' referring providers to support continuity of care.
- Provide access to community resources and connect patients with support services.
- Auxiliary in tracking program metrics and quality indicators.
- Serve as a liaison between community personnel and the interdisciplinary team at LBJ Hospital.
- Perform other duties as assigned.
- Required: Bachelor's Degree Nursing
- Required: Two years professional nursing experience to include oncology nursing (ambulatory and/or acute care)
- Preferred: Medical Oncology and Oncology Certified Nurse (OCN) certification
