About the Role

The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration-sensitive diagnoses. Serves as a patient advocate who coordinates care in conjunction with physicians from time of diagnosis or admission through discharge or up to end of life. Educates patients and their families/significant others on the health care experience; implements, coordinates, monitors, and evaluates care options, services, and resources. Links patients and families to appropriate resources as needed. Proactively enhances the patient experience clinically and in service, while assisting in quality improvement initiatives by gathering, tracking, and researching quality metrics. Monitors outcomes such as patient satisfaction, compliance with treatment regimens, and loyalty. Interacts with department nurses and may orient and train incoming clinical nurses and patient care assistants.

Responsibilities

  • Coordinate care across the continuum for patients with complex diagnoses.
  • Serve as a patient advocate, educating patients and families about the health care experience.
  • Implement, monitor, and evaluate care options, services, and resources.
  • Link patients and families to appropriate resources.
  • Enhance the patient experience both clinically and in service.
  • Assist in quality improvement initiatives by tracking and researching quality metrics (e.g., patient satisfaction, compliance with treatment regimens).
  • Interact with department nurses and orient/train incoming clinical staff.
  • Perform other duties as assigned.

Specialty-Specific Responsibilities

  • Oncology: Work with the research team to identify and navigate patients into clinical trials.
  • Metabolic and Bariatric Surgery (MBS): Serve as the primary liaison between the hospital and MBS proceduralists, adhering to MBS and MBSAQIP standards. Attend required meetings, assist with coordination of the MBS Leadership Committee, program development, accreditation, staff/patient education, clinical pathways, and monitoring patient outcomes for quality improvement.
  • Community Health: Provide case management and coordination for low-income, uninsured, and underinsured patients. Identify medical, psycho-social, and economic conditions to ensure appropriate treatment and services. Develop and deliver health-related educational programs, provide health and wellness coaching (if certified).
  • Women’s Center of Excellence (COE): Provide individualized support to expectant mothers and families throughout the perinatal journey. Coordinate care, educate patients on pregnancy and childbirth, and facilitate communication among healthcare providers. Plan and run multidisciplinary meetings and care conferences.
  • Brain Health Outreach Program (BHOP): Perform cognitive screenings, facilitate access to dementia services, develop dementia education for community and physician audiences, act as a dementia liaison, and assist in developing the CBH volunteer program.

Requirements

  • Minimum of 5 years’ experience in nursing and 3 years’ experience in the area of specialty.
  • Registered Nurse license in the State of California (current and in good standing).

Specialty-Specific Qualifications

  • Education:
    • Community Health: BSN required.
    • GI Lab: BSN required.
    • Hoag Medical Group: BSN required.
    • Women’s COE: BSN required.
  • Experience:
    • Oncology: 1+ years of oncology nursing experience; prior navigator or research experience preferred.
    • Brain Health Outreach Program (BHOP)/Memory: 5+ years’ experience in dementia, geriatrics, public health, or case management.
    • Acute Rehab: Prior navigator experience preferred.
    • Care Management: Prior sepsis RN, case manager, navigator, or health educator experience preferred.
    • Hoag Orthopedic Institute (HOI): Prior orthopedic navigator experience preferred.
  • Certifications:
    • Acute Rehab: Certified Registered Rehabilitation Nurse (CRRN) required.
    • Community Health: BLS issued by the American Heart Association.
    • Gamma Knife: ACLS and PALS certification required within 30 days of hire.
    • Neurosciences Administration/Pain Management: BLS required.
    • Women’s COE: BLS issued by the American Heart Association.
    • Breast Care Centers: BLS issued by the American Heart Association.
    • Heart Valve Clinic: BLS issued by the American Heart Association.
    • Community Health: Health coaching certification preferred.
    • Hoag Orthopedic Institute (HOI): Active NAON member and ONCB or medical-surgical certification; ACLS certification preferred.
    • Neurosciences (Stroke): SCRN, CNRN, NVRN-BC, or ASC-BC preferred.
    • Metabolic and Bariatric Surgery: Certified Bariatric Nurse (CBN) preferred.
    • Oncology Administration: Oncology Nursing Certification (OCN) preferred.
  • Language:
    • Community Health: Bilingual skills, especially Spanish, preferred.
