About the role
The Clinical Nurse is responsible for providing safe, competent quality care to a designated group of patients and significant others. This position requires a strong foundation in nursing theory and research, along with the ability to apply critical thinking skills in patient care management.
Responsibilities
- Delivers patient care based on the professional practice model of Relationship Centered Care and the nursing process.
- Affirms patient care needs and implements appropriate interventions.
- Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care.
- Manages assignments within the Care Delivery Model of Modified Primary Nursing.
- Communicates effectively with patients, families, and the interdisciplinary team.
- Manages supplies and resources to ensure patient care efficiency.
- Facilitates the admission welcome, bedside shift report, HELP, AM/PM care, 5-minute sit down, purposeful hourly rounding, medication review, discharge wrap up, and MD/RN unit collaboration.
- Escalates concerns to appropriate personnel and supports staff in using evidence-based practices.
- Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps.
- Ensures safe and efficient patient-centered care, identifies and addresses barriers and opportunities for improvement.
- Participates in quality and safety practices and compliance measures.
- Utilizes institutional information technology to document nursing practice.
Qualifications
- Graduation from an accredited Nursing program.
- Bachelor’s Degree in Nursing (BSN).
- Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year.
- Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO.
- Licensed as a registered nurse with current registration in New York State.
- Basic Life Saver (BCLS) certification from AHA.
- NRP (Labor and Delivery, NICU dept) certification.
- ACLS certification.
- PALS certification.
- Collective bargaining unit: NYSNA-MSH.
