Clinical Navigator - PRN Days | PAM Health Jupiter Rehab

About the role

As a Clinical Navigator at PAM Health, you will be the face of our hospital, meeting patients at their most vulnerable moments and guiding them toward their rehabilitation goals. You will cultivate referral relationships, manage assigned territories, and complete patient assessments while collaborating with the business development team, admissions staff, nursing, and other internal and external stakeholders to facilitate the referral conversion process.

Responsibilities

• Work under the direction of the Director of Business Development to cultivate referral relationships and manage assigned territory.
• Complete patient assessments and make sales contacts, analyzing referral and admission data from the assigned territory.
• Collaborate with the business development team, admissions staff, nursing, and other internal/external staff to facilitate referral conversion.
• Provide feedback and recommendations for program development and quality improvement initiatives related to customer service, referral/admission processes, and stakeholder satisfaction.
• Develop and maintain excellent relationships with prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives.

Education and Training

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Experience

• Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.
