Clinical Navigator Evaluator - FT Days | Sugar Land Rehab

About the role

This position supports our Sugar Land, Humble, and Houston Heights locations with Sugar Land being the primary location. PAM Health cares for chronically and critically ill patients requiring extended hospital care. Responsibilities

• Completes patient pre-admission assessments and updates
• Works collaboratively with business development team members including the Admissions Manager and admission staff, as well as nursing and other internal and external staff to facilitate the referral conversion

Requirements

• Education and Training: Degree in a health-related field from an accredited college or university preferred
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required
• Prior marketing and/or rehabilitation/LTACH experience preferred

Qualifications

• Must have a minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience

Skills

• Strong communication and collaboration skills
• Knowledge of healthcare systems and processes
