Essential Functions

  • Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plan(s) to reflect changes in patient status
  • Refers patients to other services to improve their well-being. These referrals may include community services, treatment programs, support groups, psychiatrists, or other mental health providers
  • Works with the multi-disciplinary team to develop a discharge plan in order to provide further care, services, and referrals in order to ensure continuity of care
  • Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in area(s) of responsibility
  • Performs behavioral health consultation for patients and caregivers. Conducts psychosocial assessments. Provides supportive, crisis, bereavement and other behavioral health interventions as necessary
  • Educates, communicates, and coordinates with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely discharge, and to identify and resolve delays and issues
  • Provides support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events

Minimum Qualifications

  • Master's Degree in Social Work or related field
  • Current State Licensure as LCSW, LMFT, CMHC, or Psychologist
  • Excellent written and verbal communication skills
  • Computer Skills, i.e., email, word processing

Preferred Qualifications

  • Experience with patients, families, and treatment teams

Physical Requirements

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
  • Typically includes items of varying weights, up to and including heavy items
  • Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
  • May be expected to stand in a stationary position for an extended period of time

About the Role

This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties. This position provides care appropriate to the established plan of care in the assigned patient population. This position will primarily serve the medical floor at Lutheran Hospital. The hours are Sunday-Wednesday 8AM-6PM.

Qualifications

Experience with patients, families, and treatment teams is preferred.

Requirements

Experience in crisis and brief therapeutic interventions, experience working with individuals with dementia preferred.

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$35.60 - $54.93

Schedule

Sunday-Wednesday 8AM-6PM
