Clinical Medical Social Worker - Healthy Weight and Nutrition/Bariatrics

Job Description

• Provides developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq.
• P practices professional social work and promotes advocacy in accordance with the NASW Code of Ethics for Social Work and maintains a current Ohio Social Work license.

Essential Functions

• Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health.
• Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services.
• Participates in the development and delivery of education, research and continuous quality improvement.

Education Requirement

• MSW, required.

License Requirement

• LSW, required.
• LISW, preferred.

Experience

• 2 years clinical social work experience, preferred.
