About the role

Lead and support nursing staff to deliver exceptional in-home care throughout Philadelphia County.

Responsibilities

• Lead, motivate, and mentor nursing staff to ensure high-quality patient care.
• Visit clients for clinical assessments and develop personalized care plans.
• Oversee the implementation of care plans and support staff in the field.
• Facilitate onboarding, training, and ongoing education for nurses.
• Serve as a clinical resource for staff, clients, and families.
• Coordinate in-services and professional development activities.

Requirements

• Active RN license.
• Minimum of 2 years RN experience.
• Medical-surgical experience required; home care experience preferred.
• Leadership or supervisory experience advantageous.
• Strong clinical judgment, communication, and organizational skills.
• Reliable transportation and ability to travel within the service area.
