About the role
Lead and support nursing staff to deliver exceptional in-home care throughout Philadelphia County.
Responsibilities
- Lead, motivate, and mentor nursing staff to ensure high-quality patient care.
- Visit clients for clinical assessments and develop personalized care plans.
- Oversee the implementation of care plans and support staff in the field.
- Facilitate onboarding, training, and ongoing education for nurses.
- Serve as a clinical resource for staff, clients, and families.
- Coordinate in-services and professional development activities.
Requirements
- Active RN license.
- Minimum of 2 years RN experience.
- Medical-surgical experience required; home care experience preferred.
- Leadership or supervisory experience advantageous.
- Strong clinical judgment, communication, and organizational skills.
- Reliable transportation and ability to travel within the service area.
