Clinical Manager - Home Health Registered Nurse

About the Role

Amedisys is seeking a registered nurse clinical manager to oversee quality patient care and clinical performance in a home health setting. This is an office-based, full-time day position.

Responsibilities

• Recommend employment decisions (hire, fire, promote, change status) to the director of operations.
• Ensure overall quality of patient care and clinical performance of nurses and aides.
• Organize clinical operations for the care center.
• Review service requests and determine patient eligibility/suitability for home care.
• Coordinate patient referrals and admissions with intake.
• Oversee scheduling, payor verification, and precertification requirements.
• Review staff documentation and resolve issues related to missing, incomplete, or untimely documentation.
• Assist clinicians in establishing immediate and long-term patient goals, setting priorities, and developing the Plan of Care (POC).
• Regularly evaluate patient needs and revise the plan of care in collaboration with clinicians.
• Provide direct patient care infrequently during emergencies.
• Participate in the on-call process.
• Train staff to ensure quality care, compliance, and fiscal responsibility.
• Investigate and resolve patient, family, and physician concerns.

Requirements

• Associate or bachelor’s degree in nursing.
• Current RN license specific to the state(s) of assignment (NY RN license required).
• One year of clinical RN experience in home health or hospice.
• One year of RN management experience.
• Current CPR certification.
• CHHA and OASIS experience required.

Pay

Attractive pay range of $115,000–$122,000 annually, plus a quarterly bonus program.
