Job Summary

The Clinical Manager oversees patient care coordination, supervises the clinical team, and ensures compliance with state regulations and Medicare Conditions of Participation. They collaborate with interdisciplinary teams to deliver high-quality home health services and maintain patient outcomes.

Job Duties

• Reviews referral and patient information to determine home health needs and assigns clinicians.
• Coordinates patient care, reviews services provided, and guides clinicians for quality service delivery.
• Maintains schedules, monitors patient outcomes, and produces reports for efficiency and quality.
• Collaborates with the agency director to achieve organizational goals and conducts weekly case management reviews.
• Monitors lab values, documents findings, and manages start-of-care, 485, resumption, and discharge reviews.
• Maintains telehealth processes, interprets transmissions, and follows up with patients as needed.
• Maintains procedures and protocols, researches best practices, and resolves patient and physician complaints.
• Prioritizes interventions, implements staffing guidelines, and assigns visits for per diem and part-time staff.
• Serves on-call duties, participates in quality management programs, and performs other tasks as directed.

Team Conference/Meetings

• Facilitates monthly coordination and case conferences to assess patient needs and develop individualized plans of care.
• Ensures complete admission documentation and referrals to appropriate disciplines.
• Convenes management meetings to address individual eligibility concerns and discusses case management issues.
• Facilitates monthly team meetings to share updates, discuss operational issues, and oversee bi-weekly payroll.

Personnel Management

• Screens and interviews new personnel, recommends employment, and oversees orientation and competency assessments.
• Develops and implements preceptor and mentoring environments for clinical staff.
• Completes performance evaluations, provides feedback, and addresses employee issues.

Qualifications

• Registered Nurse with current CA licensure.
• Bachelor’s degree preferred.
• Two years management or supervisory experience in health care, preferably home care.
• CPR certification.
• Licensed driver with an insured vehicle.
• Complies with professional standards and practice.
• Demonstrates excellent communication skills.

Physical Qualifications

• Ability to lift 50 lbs.
• Ability to push/pull 50 lbs.
• Ability to stoop, bend, and squat.
• Ability to walk up one flight of stairs.
• Ability to walk one mile.
• Ability to sit for one hour, minimum.
• Visual and audio acuity.
